Job Description

SUMMARY

The Program Manager position is responsible for providing strong leadership to ensure the provision of high quality among staff and across departments and programs. As part of a multi-disciplinary team, the Program Manager will travel to multiple locations to meet and work with clients to attain wellness and recovery and sustain permanent housing. The Program Manager will provide supervision and case management services to ensure housing stability, improved health outcomes, increased wellness and recovery, and community integration. This position is responsible for maintaining the highest quality provision of direct services while fostering internal teamwork and continuity of care with other service provider partners.

 

ESSENTIAL DUTIES & RESPONSIBILITIES 

  1. Ensure that services to clients are reflective of the company’s model, which emphasizes customer choice, harm reduction, and psycho-social rehabilitation
  2. Provide support to the staff for emergencies and crisis situations
  3. Be available to respond to emergency calls after work hours and weekends
  4. Assist in the screening, assessment, and enrollment of clients, and determining eligibility
  5. Develop new curriculums and program activities to meet needs of participants
  6. Ensure Case Manager enter weekly case notes on HMIS, CHAMP and complete CES intakes for new clients
  7. Ensure all clients paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases through on-going audits of electronic data bases and chart reviews
  8. Ensure housing performance standards of partners and funders are achieved, including a minimum of 80% housing retention rate
  9. Provide individual and team supervision, support, and performance evaluations for assigned staff
  10. Ensure adherence to funding source contracts and performance standards; prepare written reports on program activities as assigned and as required by funding sources
  11. Ensure program and staff follow all Policies and Procedures as outlined in contracts and grants
  12. Provide input, evaluation, and updates regarding programs as requested
  13. As needed, assist in involving clients in daily activities, budgeting, facilitating socialization, health maintenance, sense of community, and progress in recovery
  14. Maintain confidentiality of clients files per agency policies and HIPAA and follow all Policies and Procedures as outlined by HUD, DHS, HCA, LAHSA contracts or any other contracts
  15. This position will liaise with funders, other community providers and landlords to identify available units
  16. Participant in funder/program meetings to provide updates and program progress
  17. In collaboration with the Senior Director of Residential Services provide oversight and administration of program contracts to ensure successful implementation, coordination participant services, achievement of program performance measures and evaluation of services provided  
  18. Detail oriented with excellent time management organizational, written, verbal, interpersonal, computer, and cross-cultural communication skills
  19. Provide leadership and supervision to a team of volunteers and interns by demonstrating integrity, professional boundaries and providing constructive feedback.
  20. Coordinate with partners to provide counseling services and assist with client engagement
  21. Build a strong team through open communication and by collaborating on decision- making responsibilities
  22. Experience with compiling and following strict budgets

 

QUALIFICATIONS

  1. Current, valid California Driver’s with an acceptable driving record and reliable vehicle
  2. Able to transport clients in your vehicle
  3. Bachelor’s degree in related field (Social Work, Psychology, or other Human Services)
  4. Minimum of four years’ full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions
  5. Able to provide direct supervision and management of assigned staff
  6. Detail oriented with excellent time management, organizational, communication, interpersonal, and computer
  7. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers skills
  8. Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
  9. Strong interpersonal and team building skills
  10. Ability to relate effectively to diverse groups of people from all social and economic segments of the community
  11. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment
  12. Able to obtain and maintain CPR/1st Aid certification
  13. Able to work late hours and weekends
  14. Able to be on-call 24/7

Preferred Qualifications

  1. Masters in Social Work, Psychology, or other Human Services field
  2. Two years’ supervisory experience
  3. Knowledge and experience in chemical dependency and substance abuse treatment
  4. Minimum 30 WPM typing speed
